Your First Visit

WHAT TO EXPECT

Your first visit will last approximately 30 minutes to 1 hour. We request that all patients arrive 10 to 15 minutes prior to their scheduled appointment time to complete medical history and registration forms. We also request patients bring:

• Current x-rays and records from previous dental caregiver
• List of any current medications
• Dental insurance card, if insured

Any patient under the age of 18 must be accompanied by a parent or guardian.
